Comparison between transdermal buprenorphine and intravenous paracetamol for post-operative analgesia after major plastic reconstructive surgery under general anesthesia – A randomized double-blind controlled trial

Background: Adequate pain management is an essential entity in reconstructive surgery to prevent adverse physiological and psychological outcome. Transdermal buprenorphine has been mostly studied chronic cancer related but hardly studies acute are available. Aims Objectives: The aims of this study were compare post-operative relief achieved by transdermal IV paracetamol terms safety efficacy. Sedation effects also studied. Materials Methods: This a prospective, parallel group, double-blind, randomized trial. After ethics, clearance consent from 46 patients undergoing major allotted into Group B, (n=23) who received P m 6 hourly. Paracetamol was taken as active control. Standard institutional protocol for general anesthesia followed. Visual analog scale (VAS) score measured postoperatively 48 h. Diclofenac sodium the rescue analgesic. assessed Ramsay score. Results: patch (TDB) gave superior comparison intravenous h postoperatively, P≤0.0.5 pin VAS total amount analgesic required high than B (P=0.034). Hemodynamic stability better maintained with TDB minimal sedation side effect. Conclusion: (20 μg/h; 20 mg) can be safely used analgesia greater efficacy when compared paracetamol.
